v0.31.0 feat: hot memory — facts hook + recall CLI + MCP _meta + consolidate phase (#785)
* v0.31 feat(migrate): facts hot memory schema (migration v40) Phase 1 of v0.31 hot-memory. - New facts table with source_id (TEXT FK to sources, per-source isolation), kind CHECK (event/preference/commitment/belief/fact), visibility CHECK (private/world for takes-style ACL parity), valid_from/valid_until/ expired_at/superseded_by for temporal + supersession audit, and consolidated_at/consolidated_into pointing at takes(id) for the dream- cycle hot→cold bridge. - Embedding column dim resolved at migration time from config.embedding_dimensions so non-OpenAI brains (Voyage etc) work out-of-the-box. HALFVEC where pgvector >= 0.7; falls back to VECTOR with stderr warn on older versions. Matching opclass per column type (halfvec_cosine_ops vs vector_cosine_ops). - 5 partial indexes leading on source_id so every read uses the trust boundary as part of the index, not a callback. HNSW partial index excludes expired/null rows so footprint stays proportional to active fact count. - RLS DO-block matches takes pattern (Postgres BYPASSRLS gate; PGLite no-op). - v0_31_0.ts orchestrator follows v0_28_0.ts pattern — phase A asserts schema version >= 40 + facts table presence; runner owns ledger. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * v0.31 fix(migrate): add facts→sources FK explicitly via ALTER TABLE The inline column-level FK declaration on facts.source_id worked on PGLite but silently got dropped on Postgres in the v0.31 e2e run — the migration handler ran via postgres-js's `unsafe()` multi-statement path and the resulting facts table came back without the `facts_source_id_fkey` constraint. Same psql input run directly against the same database produced the FK; the difference was the unsafe() pipeline, not the SQL itself. Splitting the FK into a separate ALTER TABLE inside a DO block makes the constraint declaration explicit and idempotent: the named constraint either exists or it doesn't, the ALTER is a no-op on re-runs, and the failure mode is loud rather than silently leaving a CASCADE-less foreign key behind. Without this fix, deleting a source row leaves orphaned facts rows (test/e2e/facts-cross-source-isolation.test.ts CASCADE-on-sources- delete case caught it). With this fix the constraint is in place, the cascade fires, and both PG + PGLite e2e suites stay green. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * v0.31 fix: thread auth + sourceId, JSON-shape every error envelope Three bugs surfaced during the full e2e sweep that all trace back to my v0.31 dispatch refactor (D12/eE1) silently dropping auth threading + non-OperationError exceptions emitting plain strings: 1. **HTTP MCP transport lost ctx.auth.** Refactoring serve-http.ts to call dispatchToolCall meant auth had to come through DispatchOpts, but the field didn't exist yet. Every HTTP whoami call returned `unknown_transport` because ctx.auth was undefined. Added `auth?: AuthInfo` to DispatchOpts, plumbed it through buildOperationContext, and updated serve-http.ts:816 to pass `auth: authInfo` alongside sourceId/takesHoldersAllowList. Pinned by sources-remote-mcp e2e `whoami reports oauth transport + sources_admin scope`. 2. **Non-OperationError exceptions emitted plain strings, not JSON.** The pre-v0.31 serve-http.ts always wrapped errors in JSON envelope `{error, message}`; my dispatch refactor missed the unknown-tool + uncaught-throw paths and emitted `Error: ${msg}` text content. Every caller that did `JSON.parse(content)` (sources-remote-mcp callMcp helper at line 104) crashed with `Unexpected identifier "Error"`. Both error paths in dispatchToolCall now return JSON-shaped content matching the OperationError pattern. 3. **Files→sources FK silently lost on rewound bootstrap path.** test/e2e/postgres-bootstrap.test.ts simulates a pre-v0.21 brain by `DROP TABLE IF EXISTS sources CASCADE` which removes files_source_id_fkey while leaving files.source_id intact. The v23 migration's `ALTER TABLE files ADD COLUMN IF NOT EXISTS source_id ... REFERENCES sources(id) ON DELETE CASCADE` is a no-op when the column exists, so the FK never came back on upgrade — and any sources-remove afterward stopped cascading to files. Added a defensive `IF NOT EXISTS files_source_id_fkey ... ALTER TABLE ADD CONSTRAINT` block inside v23's handler. Pinned by `multi-source — cascade delete covers every dependent row` after running postgres-bootstrap. Plus: src/core/preferences.ts now honors GBRAIN_HOME for `~/.gbrain/migrations/completed.jsonl`. Without this, the doctor exits-0 mechanical test inherits the developer machine's stale partial-migration ledger entries (0.21.0, 0.22.4, 0.28.0, 0.29.1 prior dev work) and surfaces them as the [FAIL] minions_migration check. GBRAIN_HOME-scoped tempdir per test now isolates this state cleanly. v0.26.5 feat: destructive operation guard end-to-end (sources + pages + autopilot purge) (#600)
* feat(v0.26.5): destructive operation guard — impact preview, confirmation gate, soft-delete
Three-layer protection against accidental data loss:
1. **Impact preview**: Every destructive operation (sources remove, purge)
now shows a formatted preview of exactly what will be destroyed —
page count, chunk count, embedding count, file count — BEFORE acting.
2. **--confirm-destructive flag**: `--yes` alone is no longer sufficient
when a source has data. Must pass `--confirm-destructive` to proceed
with permanent deletion. Prevents scripted/reflexive destroys.
3. **Soft-delete with 72h TTL**: New `gbrain sources archive <id>`
hides a source from search and federation without destroying any data.
Data preserved for 72 hours. Restorable via `gbrain sources restore <id>`.
Expired archives purged via `gbrain sources purge`.
New subcommands:
- `gbrain sources archive <id>` — soft-delete (hide, preserve 72h)
- `gbrain sources restore <id>` — un-archive, re-federate
- `gbrain sources archived` — list soft-deleted sources + TTL
- `gbrain sources purge [<id>] [--confirm-destructive]` — permanent delete
Behavioral changes:
- `sources remove` with data now requires `--confirm-destructive` (not just `--yes`)
- `sources remove --dry-run` shows full impact preview without side effects
- Impact box format shows source name, id, and all cascade counts

Closes the destructive-guard posture across every gbrain destructive surface.
PR #595 cherry-pick covered the CLI source-remove path; this commit closes
the higher-velocity MCP `delete_page` agent footgun and the three internal
correctness gaps the CEO+Eng review surfaced:
- Gap 1: archived sources were not actually filtered from search. Now they
are, via `buildVisibilityClause` in `searchKeyword`/`searchKeywordChunks`/
`searchVector` for both engines.
- Gap 2: 72h TTL was honor-system. Now wired into a new autopilot `purge`
phase (9th in ALL_PHASES) that calls `purgeExpiredSources` + `engine.
purgeDeletedPages(72)`. Manual escape hatch: `gbrain pages purge-deleted`.
- Gap 3: zero tests for safety-critical code. ~30 cases now in
`test/destructive-guard.test.ts`, `test/pages-soft-delete.test.ts`, and
`test/sql-ranking.test.ts` covering the boundary truth table, JSONB→column
migration, soft-delete/restore/purge round-trip, multi-source isolation,
cascade verification, and the Q3 IRON-rule contract test.
Schema migration v33 (`destructive_guard_columns`): adds `pages.deleted_at`
+ partial purge index, promotes `archived` from `sources.config` JSONB to
real columns (`sources.archived BOOLEAN`, `archived_at`, `archive_expires_at`),
backfills any pre-v0.26.5 JSONB shape. Engine-aware: Postgres uses CREATE
INDEX CONCURRENTLY, PGLite uses plain CREATE INDEX. Forward-reference
bootstrap extended in both engines so pre-v0.26.5 brains don't crash on the
embedded-schema replay.
BrainEngine surface: new `softDeletePage` / `restorePage` /
`purgeDeletedPages` methods + `includeDeleted` flag on `getPage`/`listPages`.
MCP ops: `delete_page` rewired to soft-delete (description string updated);
new `restore_page` (scope: write) + `purge_deleted_pages` (scope: admin,
localOnly: true).
Q3 contract (eng-review lynchpin): `get_page(slug)` returns null for
soft-deleted by default; `get_page(slug, {include_deleted: true})` surfaces
the row with `deleted_at` populated. Same flag for `list_pages`. Mirrors
the search-filter contract end-to-end.
Issue 5 (eng-review): `archived` is now a real column on `sources`, not a
JSONB key. No reserved-key footgun. Faster filter. Visibility clause
compiles to a column lookup, not JSONB containment.
Verification:
- bun run typecheck: PASS
- bun run build:schema + bun run build:llms: regenerated
- targeted test runs: 90 pass / 0 fail across destructive-guard,
pages-soft-delete, sql-ranking, schema-bootstrap-coverage, build-llms
- full bun test: 16 pre-existing failures inherited from v0.26.2 (sync,
sync-parallel, queue-child-done, etc — already filed in TODOS.md as
"Fix 22 pre-existing test failures unrelated to OAuth")
CHANGELOG, CLAUDE.md (Key Files + Commands), TODOS.md updated. The plan
file at ~/.claude/plans/take-a-look-and-gentle-pine.md captures the full
review trail (CEO=C, Eng-Q3=A, Eng-Issue5=a, 8 defaults applied).

Two CI failures from the v0.26.5 ship:
1. **Tier 1 (Postgres E2E):** `E2E: Page CRUD > delete_page removes page and
others survive` failed because `delete_page` now soft-deletes (sets
deleted_at) but `getStats.page_count` was still counting all rows. The
test seeds 16 pages, deletes one, and asserts page_count is 15. Fix:
`getStats` now filters `WHERE deleted_at IS NULL` for page_count in both
engines. This matches the visibility-filter contract — soft-deleted pages
are hidden everywhere the user looks (search, get_page, list_pages, stats).
Chunks and links stay raw because they still occupy storage until the
autopilot purge phase runs.
2. **Test 2 (PGLite unit):** `multi-source-integration.test.ts:184` and
`e2e/multi-source.test.ts:274` called `runSources(engine, ['remove', X,
'--yes'])` against populated sources. v0.26.5's destructive guard rejects
`--yes` alone on populated sources and calls `process.exit(5)`, which
killed the bun test runner mid-suite (CI exit 5). Both test sites now
pass `--confirm-destructive` per the v0.26.5 contract.
Verification: 115/0 pass across destructive-guard, pages-soft-delete,
sql-ranking, schema-bootstrap-coverage, sources, repos-alias, and
multi-source-integration test files. typecheck PASS.

phases. v0.26.5 added the autopilot `purge` phase as the 9th, so:
- `test/core/cycle.test.ts:381` — `hookCalls` is now 9 (one yield per phase)
- `test/core/cycle.test.ts:392` — `report.phases.length` is now 9
- `test/e2e/cycle.test.ts:101` — same update for the dry-run E2E
The `purge` phase invocation was already visible in the failing log output:
the cycle ran 9 phases end-to-end; the test assertions hadn't been updated.
Verification: bun run typecheck PASS. cycle.test.ts: 28/0 pass.

sees one shape and the schema has another.

'default', so slug remains effectively unique within it. Step 5+ will

Search dedup now keys on (source_id, slug) instead of slug alone. Pre-
v0.17 would collapse two same-slug pages in different sources into
one, destroying cross-source recall. Codex outside-voice review flagged
this as regression-critical — this commit ships the fix plus tests
that lock the invariant in.
Dedup pipeline (src/core/search/dedup.ts):
- pageKey(r) helper — one canonical composite-key derivation. Falls
back to source_id='default' for pre-v0.17 rows so single-source
brains behave identically to before.
- Layer 1 (dedupBySource): group-by composite key.
- Layer 4 (capPerPage): count-by composite key.
- guaranteeCompiledTruth: swap scoped to matching (source_id, slug),
so wiki:topics/ai can't accidentally pull gstack:topics/ai's
compiled_truth chunk.
SearchResult type gains optional source_id — populated by SQL JOINs
in both engines, falls through as 'default' for legacy callers.
Engine SQL:
- pglite-engine.ts + postgres-engine.ts: search SELECTs add p.source_id
- rowToSearchResult (utils.ts): maps row.source_id → result.source_id
when present. Shape stays backward compatible (field optional).
Tests — 4 new in test/dedup.test.ts:
- same-slug-different-source does NOT collapse (the critical regression
guard Codex called out)
- same-slug-same-source DOES still collapse (no over-correction)
- missing source_id falls back to 'default' for pre-v0.17 compat
- compiled_truth guarantee scopes to composite key (Codex second pass
caught this specific path would leak otherwise)

Adds files.source_id + files.page_id + the file_migration_ledger
state machine that drives storage object rewrites. Each per-file
transition is its own transaction so crash-point recovery is a
ledger read, not a filesystem inspection. Codex second-pass review
flagged that "skip if already has source prefix" was an unsafe
heuristic — the ledger replaces it with explicit state tracking.
Schema:
- migration v19 (files_source_id_page_id_ledger): handler-only
(PGLite has no files table; Postgres-only gate). ADDs
source_id + page_id to files, backfills page_id from page_slug
scoped to source_id='default', creates file_migration_ledger
with PK on file_id (Codex: not storage_path_old — two sources
can share an old path during migration).
- schema.sql updated for fresh Postgres installs; file_migration_ledger
gets RLS alongside other tables.
Runtime:
- src/commands/migrations/v0_17_0-storage-backfill.ts: drives the
ledger state machine pending → copy_done → db_updated → complete.
Idempotent per row: re-running resumes from whichever state
crashed. Old objects preserved (no delete) so operators can
verify the soak window before a future cleanup release.
- phase B in v0_17_0.ts orchestrator: wires the storage backend
(Supabase/S3/local) through createStorage, runs runStorageBackfill,
reports per-state counts + first-three error details.
Tests — 13 new in test/storage-backfill.test.ts:
- pending → copy_done → db_updated → complete happy path
- 3 crash-point recovery tests (resume from copy_done, resume from
db_updated, failed rows don't auto-retry)
- already-complete rows are skipped with zero side effects
- idempotent re-upload (exists-check skips redundant upload)
- dry-run mode (no storage, reports counts without mutating)
Plus 5 new migrate.test.ts assertions for v19 structure (handler-
only, PGLite gate, source_id + page_id + ledger DDL, default-source
backfill scope, state machine values).
Full suite: 2035 pass / 3 fail (3 pre-existing PGLite infra
timeouts).
NOT in this commit (explicitly deferred):
- DROP old page_slug column — kept for backward compat until
operators have time to verify page_id everywhere.
- DROP old UNIQUE(storage_path) in favor of UNIQUE(source_id,
storage_path) — same reason, deferred to later cleanup.

* test(v0.17.0 step 9/9): full multi-source PGLite integration suite (Lane G)
End-to-end exercise of every v0.17.0 surface against real PGLite
(in-memory, fast — no DATABASE_URL needed). The migration chain
v2→v19 runs start-to-finish and the test asserts each Step's
invariants hold together.
16 new integration tests across 7 describes:
1. Migration-installed state:
- sources('default') exists with federated=true config
- pages.source_id column has DEFAULT 'default'
- composite UNIQUE (source_id, slug) is installed
2. Default-source write path:
- putPage without explicit source → source_id='default' via schema
default clause (no engine API change needed for single-source brains)
3. Composite UNIQUE regression guards (Codex-flagged):
- Same slug in two different sources coexists
- Third insert with same (source_id, slug) hits the UNIQUE constraint
4. sources CLI round-trip:
- federate / unfederate flips config.federated
- rename changes display, id stays immutable
5. Source resolution priority (integration):
- Explicit flag > env var > fallback to default
- Unregistered explicit source errors with actionable message
6. Cascade semantics:
- sources remove cascades to pages; default source untouched
7. links.resolution_type (Step 4):
- Qualified/unqualified values accepted
- CHECK constraint rejects invalid values
All 16 tests pass. Full suite: 2042 pass / 4 fail (4 pre-existing
PGLite beforeEach timeouts in test/wait-for-completion,
test/extract-fs, test/e2e/search-quality, test/e2e/graph-quality
— count fluctuated 3-5 on baseline from variance alone).
Total new tests across Steps 1-9: ~85 unit + integration tests
(sources, source-resolver, migrate v16/v17/v18/v19 structural,
link-extraction qualified wikilinks, dedup regression-critical,
storage-backfill state machine + crash recovery, full
multi-source PGLite integration).

* fix(ci): v0_18_0 orchestrator TS narrow + mechanical test ON CONFLICT
Two CI failures on PR #337:
1. tsc TS2367 at src/commands/migrations/v0_18_0.ts:190 —
after the early-return on `a.status === 'failed'` (line 179),
TypeScript narrows `a.status` to `'skipped' | 'complete'`, so the
subsequent `a.status === 'failed' ? 'failed' :` branch was dead
code and refused to compile. Dropped the redundant check.
2. E2E `file_list LIMIT enforcement` at test/e2e/mechanical.test.ts:636 —
the test pre-seeded a pages row with `ON CONFLICT (slug) DO NOTHING`
but v21 swapped the global UNIQUE for `UNIQUE (source_id, slug)`, so
Postgres rejects with "no unique or exclusion constraint matching".
Updated the conflict target to the composite key.
Tier-1 E2E had only this one failing test; everything else passed.

* test(e2e): v0.18.0 multi-source against real Postgres (v20-v23 schema + cascade + sync)
Closes the three biggest confidence gaps the author flagged in the
self-audit of PR #337:
1. No real Postgres E2E — PGLite has no files table, so v23's
files.source_id + files.page_id rewrite + file_migration_ledger
seed was NEVER executed against the real DB. This file covers it.
2. `gbrain sync --source <id>` had zero direct tests. Now has two:
one that asserts performSync({sourceId}) reads local_path from the
sources row (not the global config), one that asserts no-sourceId
falls back to the global sync.repo_path.
3. Cascade delete coverage — previously verified only pages count
after source removal. Now verifies pages + content_chunks +
timeline_entries + links + files ALL cascade-delete when a source
is removed.
6 describes, 16 tests total:
- Schema shape (fresh install): 6 tests confirming sources('default'),
pages.source_id NOT NULL with DEFAULT, composite UNIQUE pages
(source_id, slug) replaces global UNIQUE(slug), links.resolution_type
column + CHECK, files.source_id + page_id columns, file_migration_ledger
table + status CHECK.
- Composite UNIQUE semantics: 3 tests confirming same-slug in two
sources coexists (Codex-critical regression guard), duplicate
(source_id, slug) hits the UNIQUE, putPage targets default source
by schema DEFAULT.
- Cascade delete: 1 test building a fully populated source (2 pages,
chunks, timeline, links, files) then removing it + asserting every
dependent row is gone.
- Sync routing: 2 tests confirming performSync({sourceId}) reads
per-source local_path vs global config.
- Sources surface: 3 tests for federate/unfederate flipping + rename
preserving id.
- Storage backfill: 1 end-to-end test seeding ledger + running
runStorageBackfill against a stub StorageBackend, asserting
pending → complete transition and files.storage_path rewrite.
Gated by DATABASE_URL per CLAUDE.md E2E lifecycle. Each describe's
beforeAll defensively DELETEs non-default sources + file_migration_ledger
rows so reruns are hermetic (sources isn't in helpers.ALL_TABLES).
Verified: 16/16 pass on first run AND second run (residual-state fix
holds). Full E2E suite still green.

tsc rejects the direct
`(rows as { column_name: string }[]).map(...)`
cast because postgres.js RowList rows have an iterable-row shape that
doesn't overlap with the plain-object target. Standard fix: cast via
`unknown` first so the narrowing is explicit.
Verified: `bunx tsc --noEmit` clean (ignoring the pre-existing baseUrl
deprecation warning).

* fix(v0.18.0): addLinksBatch + addTimelineEntriesBatch source-aware JOINs
Batch APIs JOINed on pages.slug globally, so two pages sharing the same
slug across sources would silently fan out — addLinksBatch(['a->b']) in
a brain with 'a' in both 'default' and 'alt' wrote 2 edges instead of 1.
Same bug on addTimelineEntriesBatch.
Fix:
- LinkBatchInput + TimelineBatchInput gain optional source_id fields
(from_source_id, to_source_id, origin_source_id for links; source_id
for timeline). All default to 'default' so existing callers are
backward-compatible on single-source brains.
- pglite-engine + postgres-engine batch JOINs now composite-key on
(slug, source_id). Postgres adds 3 more unnest arrays for links + 1
for timeline — still one bind per column, no 65535-param cap risk.
- LEFT JOIN for origin pages also source-qualified so frontmatter-
provenance edges don't cross-pollinate across sources.
Regression coverage:
- test/pglite-engine.test.ts: 5 new tests covering default-path isolation,
explicit alt-source writes, and cross-source edges.
- test/e2e/multi-source.test.ts: 4 new tests against real Postgres so
postgres-js's unnest() bind path is exercised (structurally different
from PGLite's).
Gap #4 from the PR self-audit — latent bug, not previously reachable
because every existing caller wrote to the default source only.
