Brings in v0.42.58.0 (provider-agnostic AI gateway — env clobber, base-URL
/v1 normalization, embedding-dim validation; #1249 #1250 #1292 #2271 #2209).

Conflicts resolved (version trio + TODOS only; AI-gateway code auto-merged):
- VERSION / package.json: kept 0.43.0.0 (Cathedral 1 — higher semver wins).
- CHANGELOG.md / TODOS.md: kept both sides — ours (0.43.0.0) on top, master's
  0.42.58.0 entry / provider-agnostic follow-ups below.

Verified: 3-line version audit agrees on 0.43.0.0; bun.lock unchanged;
bun run typecheck clean.
